Step-Down Permanent Supportive Housing

LifeArk Tyler (LAT) in the city of El Monte will provide permanent supportive housing and ongoing intensive case management (ICMS) services to medically vulnerable homeless individuals stepped down from Illumination Foundation’s Recuperative Care Services (IFRC). LifeArk Tyler will feature two micro-community units and a smaller unit for support staff - IF will provide ICMS support including life skills classes, SUD groups, and connections to community resources essential to long-term housing and health stability.

Following an award from Los Angeles County Homeless Initiative’s Homeless Innovation Challenge, LAT will be LifeArk’s first pilot project in the County of Los Angeles.

Illumination Foundation

IF is a nonprofit organization based in Orange County, California dedicated to breaking the cycle of homelessness. IF provides comprehensive housing, health, and supportive services for the most vulnerable homeless individuals and families in LA, Orange, and San Bernardino Counties - IF is a national leader in recuperative care services.
